Youth Ministry Outing – Minor Participants
Identification of the Organization
La Fabrique de la Paroisse de St. Ignatius of Loyola (hereinafter referred to as “the Church”), located at 4455 West Broadway, Montréal, QC, H4B 2A7, organizes youth activities through its ministry IgNation (hereinafter referred to as the “Activities”).
The Activities may include, but are not limited to, off-site outings, recreational activities operated by third parties, physical activities, group events, and, where applicable, transportation arranged by the Church.
For reference, the outing prepared for this retreat constitutes one such Activity; however, this acknowledgment applies to all similar youth ministry activities unless otherwise specified.
Inherent Risks
I acknowledge and understand that participation in the Activities involves inherent risks which may result in bodily injury, illness, property damage, serious injury, or, in rare cases, death.
These risks may include, but are not limited to:
Risks associated with physical and recreational activities operated by third parties;
Falls, collisions, slips, or contact with equipment or other participants;
Risks related to off-site group activities and public venues;
Allergic reactions, pre-existing medical conditions, or physical limitations;
Fatigue, dehydration, dizziness, or other health-related factors;
Acts or omissions of other participants;
Unforeseen accidents despite reasonable supervision.
I understand that this list is not exhaustive and that unknown risks may arise.
Transportation (When Applicable)
I understand that some Activities may involve transportation organized or coordinated by the Church, including travel by private vehicle, rented vehicle, bus, or other means.
In such cases, I acknowledge that transportation carries inherent risks. I agree that the Church, its clergy, employees, ministry leaders, volunteers, drivers, and representatives shall not be held liable for incidents occurring during transportation, except in cases of gross negligence or intentional misconduct.
For Activities where transportation is not provided by the Church, I understand that parents/guardians are solely responsible for arranging and supervising transportation to and from the event location.
Informed Consent and Release of Liability
I, as parent or legal guardian, voluntarily and knowingly consent to the participation of the Minor Child(ren) under my care in the Activities organized by the Church.
To the fullest extent permitted by applicable law, I release, indemnify, and hold harmless La Fabrique de la Paroisse de St. Ignatius of Loyola, its clergy, employees, youth ministry leaders, volunteers, agents, and representatives from any and all claims, liabilities, damages, or losses arising out of or related to my child’s participation in the Activities, except in cases of gross negligence or intentional misconduct.
I confirm that I am responsible for determining that my child is physically and mentally fit to participate in the Activities.
I authorize Church representatives to secure emergency medical treatment for my child if deemed necessary, including transportation to a medical facility. I understand that I am responsible for any medical expenses incurred.

By signing below, I confirm that I have read and understood this Acknowledgment and Assumption of Risks. I acknowledge that I am the legal parent or guardian of the above-named minor and that I am signing freely and voluntarily.
